The University of Arizona is committed to protecting employees from risks related to exposure to uncontrolled hazardous energy.
This program outlines the procedures, responsibilities and training required under Arizona Division of Occupational Safety and Health (29 CFR 1910.147) to ensure compliance and protect our university community.
EHS provides support for all key requirements of lockout/tagout (LOTO):
- Creating hazardous energy source inventory
- Writing LOTO energy control procedures (ECPs)
- Assessing and inspecting ECPs and equipment
- Training for authorized, affected, and other workers
LOTO Coverage
Lockout/Tagout, or "LOTO," is a applicable to all university workers who may be authorized to perform LOTO, may be affected by LOTO, or those who may be in the area of LOTO. All equipment, machines, and processes that have uncontrolled hazardous energy sources apply.
Energy sources include but are not limited to: Electrical, Pneumatic, Hydraulic, Gravity/stored, Thermal, Mechanical, Chemical.
LOTO Written Program
This written program details the LOTO program requirements, roles and responsibilities, and provide templates to support compliance and safety.
LOTO Training
Initial, periodic, and annual training are required for any authorized and affected workers. EHS can provide onsite training upon request.
Inventory & ECPs
EHS provides an equipment and hazardous energy source inventory, energy control procedure (ECP) template, training record form, emergency device removal form, and more to all university entities. Complete the chemical inventory and send to ehs-help@arizona.edu to initiate the service and receive orientation.
